Path are recruiting a Digital & Transformation Managers for PCN projects in the South East.
The Digital & Transformation Lead will work with clinical and operational teams within the PCN's to improve patient access, staff and patient satisfaction, and the efficiency and sustainability of the PCN and practice services.
Transformation
GP practices are under significant pressure – capacity is not meeting demand - and therefore need support to successfully make changes and improvements to workflows, alongside improving access to care and support for patients. Digital transformation is essential to support increased access to care for patients. It should enable greater productivity meaning staff can work more effectively, deliver care at-scale and support the overall sustainability of general practice services.
Role outline
Working closely with staff onsite & remotely to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. It is likely to be necessary for the post holder to liaise with other PCNs Digital & Transformation Leads, key individuals within ICB's, including IT / Digital leads, and other community stakeholders.
Using data, you will lead on Quality Improvement (QI) processes and coordinate and directly support the delivery of transformation activity in AMP through optimising digital tools, operational processes and pathways and finding new ways of working. You will also support the General Manager and GP Partners in identifying costs and financial implications of utilising any digital tools.
Working with the clinical and operational teams within a Primary Care Network (PCN) to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration,
resource and tools to deliver changes in a collaborative way. You will support this vision to be developed in alignment with ICS strategy.
You will plan, coordinate and directly support delivery of transformation activity in general practice within a PCN – with a view to improving patient access,
staff and patient satisfaction, and the efficiency and sustainability of general practice services. Working closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for
improvement and then support these to be delivered. Developing strong relationships with PCN member practices you will support the planning and delivery of improvements that will provide benefits to patients and staff.
Digital Access
You will support the wider and/or improved utilisation of digital tools such as:
- Online consultations
- Online registrations
- Website use
- App use (NHS App and other available healthcare apps)
- Cloud Telephony applications
- Business Intelligence tools
This includes reviewing the use of digital tools, as well as identifying and addressing any challenges/inefficiencies that impact on staff workload and patient experience.
The core objective of the PCN Digital and Transformation Lead role is to support the PCN and its practices to deliver ongoing improvement to services. The role will require experience in business / service improvement with expertise in
transformational change and a good understanding of general practice systems and processes, including broad familiarity with general practice digital systems and how they can be used to deliver improvement.
Ideally, you will have :-
- An interest in and understanding of health care and health issues
- An interest in improving the patients experience / care / safety through training from audits or quality improvement projects.
- Experience of championing diversity and inclusion and promoting actions to make improvements to the experience of diverse groups.
- Use of Clinical Data Entry Templates / Protocols / Pre-sets
- Online consultation tools
- New Patient Online Registration
- Hands on experience with primary care clinical systems (e.g EMIS or System One, Ardens, Apex)
- Video consultation tools
- Understanding of financial processes and practices
It is essential that you have:
- Degree or other evidence of academic study, research or significant initiatives in transformation change using digital as an enabler.
- Experience of working on projects, including successfully delivering change and improvement programmes.
- Previous experience in project management and planning through entire project lifecycle from initiation to delivery
- Experience of interacting with internal and external stakeholders at a variety of levels and across functions, managing expectations and ensuring high levels of satisfaction.
- Excellent communication and presentation skills, with the ability to clearly communicate complex issues to a variety of audiences
- Ability to work effectively under pressure, within strict time constraints and must be able to manage multiple projects in a fast-paced, time sensitive environment.
- Familiarity with and technical knowledge of IT products such as primary clinical systems and digital tools
